Personal Care Management.
Oast & Hook can offer assistance as needed to senior citizens or disabled persons in a professional manner while maintaining dignity for the elderly and their families. We are an advocate for our elderly clients and strive to maintain a continuum of care to prevent unnecessary hospitalizations and costly crises. As a liaison between the family and the health-care community, we offer security and 24-hour availability to reduce stress and costs to clients.

When the activities of daily living become an effort or unsafe for our frail elders, the chronically ill, and the physically disabled, the challenges of our care manager begin. It is the goal of the care manager to find the support for the endangered groups to live in the least restrictive community setting of their choice through the enablement of community, family and personal resources. Our Care Manager works hand in hand with professional practitioners such as lawyers, physicians, accountants, financial planners, health care and long term care agencies as a team to create a plan for the individual.

On a daily level, our care manager may pay bills for the client who can no longer manage a checkbook, or go to the emergency room in the middle of the night to make sure the hospital has all current and pertinent information on the patient. Being in an emergency room alone can be a scary experience for anyone, but for the memory deficit person, it can be catastrophic; our care manager will be that familiar face at times like this to try to keep them calm. By Oast & Hook keeping the distant relative apprised of the client's condition, it is reassuring and comforting when they can't be with their relative. Some elders have either no family or no family in the area. That's our care manager becomes the client's advocate, or the contact to the outside world for the institutionalized client.
As an Elder Law firm, Oast & Hook provides a continuity of services within a team framework. Our attorneys provide an estate plan which is researched and reviewed by our in house accountant and trust officer. Planning for the future, both financially and legally, is key in developing a plan of care. Our care manager strives to implement community resources when necessary and then will monitor those services to keep the level of care consistent.
