Glossary

Care Management
All encompassing management between the care manager and the client. The client and the care manager discuss the needs of the client and to what extent the care manager is to intervene. This can be in areas of financial assistance (bill paying), medical (escorting to doctor's appointments or setting up in-home health care), liaison between distant family and client, or as advocate between client and living facility.

Advocate
One that pleads the cause of another.

Liaison
A close bond or connection.

Continuum
A continuity and consistency of care information to family, professionals, and community resources.

Five ADL's (Activities of Daily Living)

  • Toileting
  • Bathing
  • Dressing
  • Transferring
  • Eating

Community Resources
Any agency, company, facility, or service in the community to give assistance in supported independence to those at risk.

Home Health Care
In-home assistance with ADLs; in-home visits by a nurse, and/or physical therapists.

Assisted Living Facility
A facility to assist with the ADLs and where residents can retain a degree of independence.

Nursing Home
A privately operated establishment providing rehabilitation or skilled nursing care for persons who are unable to care for themselves properly.
